In 2017, ACHC established a new department to provide a Community Health Worker (CHW). Joan Field is our CHW, and extends the Clinic's care into the Patient's home.
Joan is also an Oregon Health Plan (OHP) Assister, and can help you apply for OHP.

The Asher Community Health Center’s (ACHC) Quality Improvement Plan seeks to enhance the health of our patients and communities and is designed to help us achieve our vision and mission to continuously improve the excellence of our services, processes, provider and support staff performance, the quality of our reporting
and our decision-making skills.
ACHC’s Quality Improvement program monitors quality through ongoing assessment of and response to measures of quality of care,
patient safety, patient satisfaction, organizational efficiencies, staff feedback,
Board of Directors input,
and financial measures.

Telemedicine increases access to care. Distance and travel time between patients and care providers can limit access to care. Fortunately, telemedicine can overcome geographic barriers to healthcare, especially for frontier-rural patients and providers. Telemedicine can be particularly beneficial for patients in medically underserved communities and those in rural geographical locations where travel to healthcare resources can delay care.
When its needed, you will find Doxy.Me simple and convenient to use. And there is no fee to use the Doxy.Me connection.
No need to download software or create an account. Just use a browser on a computer or device with a camera and microphone.
When you have a Tele-Health appointment, you can see your Provider from anywhere. Just click their personalized room link that you'll be provided, (example: doxy.me/YourDrsName) and you are able to join them for a video call.
Telehealth has been shown to overcome barriers to health services caused by distance between patient and provider, access to reliable transportation, fragmentation of care due to gaps in time between appointments, and lack of available providers.
