In 2017, ACHC established a new department to provide a Community Health Worker (CHW). Joan Field is our CHW, and extends the Clinic's care into the Patient's home. To learn more about the CHW program, click here.
Quality Assurance ~ Quality Improvement:
The Asher Community Health Center’s (ACHC) Quality Improvement Plan seeks to enhance the health of our patients and communities and is designed to help us achieve our vision and mission to continuously improve the excellence of our services, processes, provider and support staff performance, the quality of our reporting
and our decision-making skills.
ACHC’s Quality Improvement program monitors quality through ongoing assessment of and response to measures of quality of care,
patient safety, patient satisfaction, organizational efficiencies, staff feedback,
Board of Directors input,
and financial measures.
To provide you with the best possible care, we ask you to share your thoughts and opinions periodically with Surveys. We want to know what is important to YOU.
