ACHC is a Federally Qualified Health Center (FQHC). This means that it receives some of its funds from the federal government in order to provide low cost health care to low-income people. This does not mean that ACHC is a free clinic. The federal government expects us to charge for services. Any insurance will be billed. However, charges for low income patients are reduced based on income and family size.
To see if you and your family qualify for reduced fees please make an appointment by calling 541-763-2725.
If you can, bring the completed form (download button is below) to the Clinic and request an evaluation of your status.
