CHW:     The CHW Program at Asher CHC is staffed by an Oregon state-registered Community Health Worker (CHW).

The CHW program is designed to bridge healthcare gaps and to help coordinate care so our Wheeler County residents can receive high quality and targeted healthcare based on their specific conditions or challenges. 

Community Health Work access is through Provider Referral, or by direct contact from the patient or their family with the CHW. 

The Oregon Health Plan (OHP) provides Oregonians access to health care.   OHP covers regular check-ups, prescriptions, mental health care, addiction treatment, and dental care. Your income will determine if you are eligible for OHP.   There are also state subsidies to help people who make too much money for the Oregon Health Plan to pay for private insurance.
